  • College Applications: If your Senior is still planning to apply to a college, please be sure they have submitted the following forms in order for the counselors to write letters of recommendation:

    • A current Student Resume
    • A completed Parent Questionnaire
    • For Seniors who have a last name that are between L - Z, you need a completed CHS Counselor Recommendation Questionnaire
    • All of these forms can be found in the Counseling Office or here under Forms on the CHS website.

  • IMPORTANT: We require TEN working days to complete a student’s admission credentials (including counselor recommendations and transcripts).

  • A Transcript Request Form is available on the chsvb.org website and in the Counseling Office. You must complete a form for EACH college and scholarship before the registrar, Mrs. Schrader, can prepare a transcript.
  • When requesting a letter of recommendation from teachers, please do so in person and provide them with the yellow Recommendation Request Form found in the Counseling Office. Please make sure to give them at least two weeks to write the letter.
