A student’s personal sense of well-being and level of achievement depend upon their health and wellness.

Catholic High School is fortunate to have a full-time registered nurse on duty during school hours. She is available for the support of the well-being of student’s health, the assessments and care for chronic and acute illness, and first aid for school-related injuries. Together, you, your child, and the school nurse can build a relationship to benefit your child.

Our nurse will also work with you to develop and implement a plan for those children who have ongoing health issues.

IMPORTANT REMINDERS

  • Each family must fill out medical information pertaining to their child through Magnus Health. 
    If you have not done so already, please access the Magnus Health Student Medical Record for your child/children by accessing the MEDICAL link under the STUDENT tab in Parents web. This will automatically log you in to the “Magnus Health Portal.”

  • If your child needs to take medication throughout the school day, parents must complete the Request for Medication Administration form and turn it into the nurse. 
    *This is required for all Over-the-Counter and Prescription medications.

  • If your child self carries medication such as an Epi-pen or Inhaler please make sure the self carry form is filled out and the nurse and student have a copy.

  • If your child has Asthma, Allergies, Seizures, or Diabetes please make sure an Action Plan is filled out and the nurse and student have a copy.
